Health care is easier with someone beside you.

The health system is hard to navigate — especially if English is not your first language, if you have just moved counties, or if you are managing care for someone else. Our community health workers close that gap.

We are not a call center. You are assigned a health worker who learns your situation and stays with you: enrolling in a plan, getting to appointments, interpreting with your provider, replacing a lost Medi-Cal card, finding housing or food assistance. We meet you by phone, in person, virtually — and at the hospital if that is where you are.

Everything we do is at no cost to Medi-Cal members. There is also no cost to a doctor, clinic, or facility to have a designated community health worker team for their patients.
