Young athletes with Medi-Cal through LA Care or Molina can receive a free Walmart e-gift card after completing a short health questionnaire with one of our community health workers.

Built for teams, clubs, and school programs.
An athlete, for this program, means:
- A child enrolled in a school or sports program with a coach or trainer — not casual backyard play.
- Covered by Medi-Cal through LA Care or Molina.
- Any age, any sport. A parent or guardian takes part in the questionnaire.
We work with organizations across Los Angeles County and San Diego County, and with programs in the High Desert. Coaches and program directors: contact us and we will walk your families through it.

We check eligibility
The athlete needs Medi-Cal through LA Care or Molina. We can confirm the plan for you — a parent or guardian just needs to be on the call.
A short health questionnaire
Done by phone with one of our community health workers, or by scanning the QR code on our flyer. It takes just a few minutes.
The e-gift card
Once the questionnaire is complete, the family receives a free Walmart e-gift card. There is no cost to the family, the coach, or the club.
