• To determine whether you qualify for community health worker services and connect you with a health worker.
• To help you enroll in, transfer, or maintain health coverage, and to assist with applications you ask us to help with.
• To follow up with you about your care, appointments, and program participation.

Who we share it with

• We share your information only as needed to provide the services you asked for — for example, with your health plan when we help you enroll — and as required by law.
• We do not sell your personal information.
